KẾT QUẢ ĐIỀU TRỊ U GAN DI CĂN TỪ UNG THƯ ĐẠI TRỰC TRÀNG BẰNG ĐỐT SÓNG CAO TẦN

Nghiên cứu nhằm mô tả đặc điểm hình ảnh và kết quả sớm của đốt sóng cao tần (RFA) trong điều trị u gan di căn từ ung thư đại trực tràng tại Bệnh viện Đại học Y Hà Nội. Nghiên cứu mô tả trên 25 người bệnh (NB) với 38 u gan di căn, thực hiện từ 01/2024 đến 04/2026. Đáp ứng điều trị được đánh giá bằng cộng hưởng từ hoặc cắt lớp vi tính theo tiêu chuẩn mRECIST.
Tuổi trung bình 56.7 ± 12.7; tỷ lệ nam/nữ tương đương. Tổn thương chủ yếu nằm ở hạ phân thùy VI (39.5%), kích thước trung bình 17.8 ± 6.0mm, đa số người bệnh có 01 u (64%).
Theo tiêu chuẩn mRECIST, tỷ lệ đáp ứng hoàn toàn tại chỗ đạt 84.2% sau 3 tháng. Nhóm tổn thương ≤20mm có xu hướng đạt tỷ lệ đáp ứng hoàn toàn cao hơn nhóm >20–30mm (91.7% so với 71.4%, p = 0.17). Các biến chứng chủ yếu nhẹ và thoáng qua, không ghi nhận biến chứng nặng liên quan thủ thuật theo phân loại CIRSE.
Kết luận: Đốt sóng cao tần là phương pháp an toàn, ít biến chứng, cho kết quả sớm khả quan trong kiểm soát tại chỗ u gan di căn từ ung thư đại trực tràng.
