KHI TỔN THƯƠNG DẠNG NANG KHÔNG PHẢI LÀ NANG: SARCOMA PHÔI KHÔNG BIỆT HÓA CỦA GAN

Sarcoma phôi không biệt hóa của gan (Undifferentiated Embryonal Sarcoma of the Liver – UESL) là một khối u trung mô ác tính hiếm gặp, chủ yếu gặp ở trẻ em và rất hiếm ở người trưởng thành. Do các đặc điểm lâm sàng, hình ảnh học và mô bệnh học không đặc hiệu, UESL rất dễ bị chẩn đoán nhầm với các tổn thương lành tính hoặc các khối u khác của gan, dẫn đến trì hoãn điều trị. Chúng tôi báo cáo trường hợp bệnh nhân nữ 33 tuổi nhập viện vì đau bụng kéo dài. Trên siêu âm và chụp cộng hưởng từ cho thấy một tổn thương dạng nang lớn ở gan, ban đầu được chẩn đoán là tổn thương sán lá gan và điều trị thuốc kháng ký sinh trùng 1 tháng kết hợp dẫn lưu nang gan. Tuy nhiên, tổn thương hầu như không thay đổi trên hình ảnh kiểm tra sau điều trị. Bệnh nhân được chỉ định phẫu thuật cắt gan trung tâm, kết quả mô bệnh học sau mổ ban đầu hướng đến sarcoma màng hoạt dịch. Sau phẫu thuật, bệnh nhân không điều trị bổ sung mà tự sử dụng thuốc nam tại nhà. Năm tháng sau, bệnh nhân tái nhập viện với triệu chứng đau bụng và khó thở nhiều, trên các phương tiện chẩn đoán hình ảnh ghi nhận tổn thương tái phát tại gan và di căn phúc mạc. Hội chẩn Hội đồng đa chuyên khoa được đề nghị xét nghiệm hóa mô miễn dịch bổ sung trên mẫu bệnh phẩm phẫu thuật trước đó, kết quả chẩn đoán xác định là tổn thương UESL. Trường hợp này nhấn mạnh tầm quan trọng của việc cân nhắc UESL trong chẩn đoán phân biệt các tổn thương gan không điển hình, đặc biệt khi không đáp ứng điều trị thông thường, và vai trò thiết yếu của tiếp cận đa chuyên khoa trong chẩn đoán và điều trị UESL.
