Imaging Features and Vascular Anatomical Variations of the Mesopancreas on Multidetector Computed Tomography: A Retrospective Descriptive Study from Bach Mai Hospital

Abstract

The mesopancreas is a fibro-neurovascular tissue region posterior to the pancreatic head and uncinate process and is closely related to tumor spread and surgical planning for pancreatic head disease. Accurate depiction of its imaging features and vascular variants may improve preoperative assessment. Objective: To describe the morphology, imaging parameters, and vascular distribution of the mesopancreatic region using multidetector computed tomography (MDCT) with three-dimensional reconstruction. Materials and methods: This retrospective cross-sectional study included 129 patients who underwent contrast-enhanced abdominal MDCT at Bach Mai Hospital between October 2025 and May 2026. Images were reviewed on PACS and Syngo.Via to assess morphology, volume, attenuation, dimensions, and vascular structures within the mesopancreatic region. Results: The mean age was 52 ± 18 years. The mesopancreatic region most commonly showed a bell-shaped (51.94%) or polygonal (48.06%) configuration. The mean volume was 6.48 ± 4.44 cm³, and the mean attenuation was -12.64 ± 29.39 HU. Vascular structures were identified in 99.22% of cases. The IPDA and IPDV were the most frequently identified vessels, observed in 95.35% and 86.82% of cases, respectively. The IPDA most commonly shared a common trunk with the first jejunal artery (61.79%), whereas the IPDV most frequently drained into the first jejunal venous trunk (55.35%). Conclusion: MDCT with three-dimensional reconstruction enables visualization of the mesopancreatic region and its vascular variants. The IPDA and IPDV are consistent anatomical landmarks that may support preoperative evaluation of pancreatic head disease.
