Factors Associated with Bone Metastases on Bone Scintigraphy in Patients with Lung Cancer

Abstract
Objective: To describe the characteristics of bone scintigraphy lesions and analyze several factors associated with bone metastatic lesions in patients with lung cancer. Subjects and Methods: A retrospective cross-sectional analytical study was conducted on 459 patients with lung cancer who underwent bone scintigraphy at the Nuclear Medicine and Oncology Center, Bach Mai Hospital from September 2025 to February 2026. Clinical, paraclinical, and imaging characteristics were collected and analyzed using univariate and multivariate logistic regression analyses. Results: The mean age of the study population was 64.3 ± 9.98 years, with males accounting for 69.1%. The detection rate of bone metastases on bone scintigraphy was 34%. The most common sites of lesions were the lumbar spine (26.6%), ribs (23.9%), thoracic spine (17%), and pelvis (14.6%). In multivariate logistic regression analysis, bone pain and disease stage were independently associated with suspected bone metastatic lesions on bone scintigraphy (OR = 15.116; p < 0.001 and OR = 1.372; p = 0.028, respectively). Conclusion: Bone scintigraphy is a valuable method for detecting bone metastatic lesions in patients with lung cancer, particularly in those with advanced-stage disease and bone pain symptoms, thereby supporting disease staging and treatment planning.
