Tóm tắt

Mục tiêu: Mô tả đặc điểm lâm sàng, xét nghiệm và hình ảnh cộng hưởng từ của ung thư đường mật trong gan típ ống nhỏ.


Đối tượng và phương pháp nghiên cứu: Nghiên cứu mô tả cắt ngang, hồi cứu, thu thập dữ liệu từ 30 bệnh nhân được chẩn đoán ung thư đường mật trong gan típ ống nhỏ tại Bệnh viện Bạch Mai (1/2021 -11/2025).


Kết quả: Tuổi trung bình 57,7 ± 10,7;cao nhất ở nhóm tuổi 50–70 (66,7%), nam chiếm 56,7%. Thường không có triệu chứng (43,3%), đau bụng (33,3%), vàng da (16,7%). CA 19-9 tăng 30,0%, HBsAg dương tính 33,3%.


Trên cộng hưởng từ, thể khối chiếm ưu thế (96,7%), kích thước ≥ 5 cm chiếm 53,3%, bờ rõ 73,3%, co kéo bao gan 60,0%. Tổn thương giảm tín hiệu trên T1W (93,3%), tăng tín hiệu trên T2W (96,7%), hạn chế khuếch tán (100%). Dấu hiệu “target sign” gặp ở 36,7% trên T2W và 53,3% trên DWI. Ngấm thuốc đặc trưng gồm ngấm viền thì động mạch (83,3%), tăng dần ở thì tĩnh mạch (93,3%); thì muộn ngấm tăng dần vào trung tâm (76,7%) và ngấm duy trì (20%), thải thuốc hiếm (3,3%). Xâm lấn mạch 3,3% và di căn hạch 30,0%; đa số không giãn đường mật (83,3%).


Kết luận: Ung thư đường mật trong gan típ ống nhỏ có biểu hiện lâm sàng nghèo nàn, xét nghiệm không đặc hiệu. Đặc điểm hình ảnh cộng hưởng từ có giá trị trong định hướng chẩn đoán.
