DIAGNOSTIC VALUE OF 18F-FDG PET/CT, CT, AND CA 12-5 IN RECURRENT OVARIAN EPITHELIAL CARCINOMA

Abstract
Objective: To evaluate the value of 18F-FDG PET/CT, CT, and serum CA 12-5 in the diagnosis of recurrent ovarian epithelial carcinoma.
Subjects and Methods: A retrospective and prospective descriptive study was conducted on 74 patients at K Hospital (Tan Trieu campus) from January 2023 to August 2024. Patients had been diagnosed with ovarian epithelial carcinoma, were disease-free for at least 6 months after treatment, were suspected of recurrence, and underwent PET/CT for assessment.
Results: The mean age of the patient group was 59.2 ± 8.5 years. The most common method for detecting suspected recurrence was elevated serum CA 12-5 (≥ 35 U/ml), observed in 69 patients (accounting for 93.2%). Peritoneal metastases were detected by PET/CT in 40 patients (59.7%) and by CT in 22 patients (51.2%). PET/CT showed a sensitivity of 97.1%, specificity of 80%, accuracy of 95.9%, positive predictive value (PPV) of 98.5%, and negative predictive value (NPV) of 66.7%. In comparison, elevated CA 12-5 had a sensitivity of 92.6%, specificity of 20%, accuracy of 86.5%, PPV of 94%, and NPV of 16.7%; while CT had a sensitivity of 62.3%, specificity of 60%, accuracy of 62.1%, PPV of 95.6%, and NPV of 10.3%.
Conclusion: 18F-FDG PET/CT is a superior method for diagnosing recurrence in ovarian epithelial carcinoma compared to conventional methods such as CT and serum CA 12-5 testing.
